Sean Dyche made 10 changes to the Burnley side that drew 1-1 against Wolves in the Premier League on Sunday with former England international Joe Hart handed a start in goal, while Danny Drinkwater was given a debut.
Sunderland boasted eight changes from their 3-1 League One win against AFC Wimbledon as out-of-favour striker Will Grigg was given a start and Jack Baldwin came in for his first appearance of the season.
Dwight McNeil hit a seventh-minute effort just wide, before the Black Cats responded through Grigg as he fired just over the crossbar at the opposite end.
However, it was the hosts who took the lead in the 11th minute when Jay Rodriguez was played in by Matej Vydra and coolly finished from 10 yards for his first goal in his second spell with the Clarets.
Burnley somehow failed to extend their lead in the 27th minute when Aaron Lennon saw his effort rebound off the post from inside the area and Vydra could only hit the rebound off the opposite upright with the goal at his mercy.
The Clarets were made to pay for spurning those two opportunities when Sunderland equalised on 35 minutes when a deflection fell kindly to Grigg in the middle of the area and he finished well from close range.
Things went from bad to worse for the hosts early in the second half when Sunderland scored two goals in the space of three minutes.
On 47 minutes, Tom Flanagan pounced to head in from close range when Hart flapped at Lynden Gooch’s shot, before Luke O’Nein’s short corner was rolled to George Dobson just inside the area and he hit a right-foot effort into the bottom corner.
Burnley almost got a foothold in the game on the hour when Kevin Long met a Ben Gibson lifted into the middle from the right but saw his bullet header impressively pushed away by Lee Burge down to his right.
Chris Wood and Matt Lowton were both thrown on by Dyche to try and get the hosts back into the contest.
However, Sunderland held on fairly comfortably to ensure they will be in the hat for the third-round draw.
